Please come prepared with the following:

Photo ID

Proof of Income

Please bring one of the following: 2 consecutive pay stubs or a bank statement with direct deposit or disability or SSI information. 

Insurance card (if you are covered by insurance)

Social Security Card

Proof of custody if you are not the biological parent. Must be court documents showing guardianship or child custody. 

Medical Marijuana Card

Additional documents needed for BIP:

1. The Probable Cause Affidavit or Police Report to explain the incident.

2. The Service documents for any and all current or past Victim Protective Orders.

3. Probation Agreement, if applicable.
